Instructions for making a donation request. Due to the volume of worthy requests that we receive annually, we can only consider written requests on your letterhead to the following address:

Each request needs to include a brief answer to the following areas:
· Name of Organization – needs to be a 501(c)3 non-profit organization
· Mission of the organization
· Amount requested
· Reason for the request
· Benefit to our community
· Date donation is needed
· Contact information of responsible individual (Name, phone number, email)
The Donations Committee is made up of City Mill Team Members who review the many worthy requests on a periodic basis.
We apologize in advance that we cannot get back to every request. If you do not hear from us within a month of your request, please know that we have considered your request but cannot fulfill all worthy causes and need to decline your request.
